Why Acne Scar Treatment Is Never One-Size-Fits-All
True acne scars form when inflammation disrupts the skin's normal healing process and collagen is lost or produced abnormally. Most facial acne scars are atrophic — meaning they sit below the surrounding skin — but raised hypertrophic or keloid scars can also develop.
Depressed acne scars are commonly described as ice-pick, rolling and boxcar scars. Many people have more than one type on the same face, which is one reason combination treatment is so common.
Treatment planning also depends on skin tone, pigmentation history, scar age, previous treatments and whether active acne is still creating new inflammation. Improving scars while new scars are still forming is rarely the smartest sequence.
Identify the Scar Before Treating It
Ice-Pick, Rolling & Boxcar Acne Scars
Search terms such as “pitted acne scars” describe several different scar shapes. Their depth and structure influence which treatment can realistically improve them.
Ice-Pick Scars
Ice-pick scars are narrow openings that extend more deeply into the skin. Because the visible opening can be small while the scar extends downward, broad resurfacing alone may not fully address a very deep ice-pick scar.
Important: selected deep ice-pick scars may require medical techniques such as TCA CROSS or punch procedures before or instead of generalized resurfacing.
Rolling Scars
Rolling scars create broad, shallow depressions that can make the skin look wavy under directional light. Collagen-remodeling treatments may improve selected rolling scars.
Deeply tethered rolling scars can be held down by fibrous bands and may require medical subcision before resurfacing can fully address the surface contour.
Boxcar Scars
Boxcar scars are round or oval depressions with more defined edges. Shallower boxcar scars may respond well to fractional resurfacing and RF microneedling, while deeper scars can be more resistant.
Treatment depth and edge definition help determine whether CO₂, RF microneedling, non-ablative fractional laser or referral for a focal scar procedure makes the most sense.
Hypertrophic & Keloid Scars
Raised acne scars are biologically different from pitted scars because excess scar tissue is present rather than a collagen deficit. They should not simply receive the same aggressive collagen-induction treatment used for depressed scars.
Raised or keloid-prone scars may require medical evaluation and treatment such as injections or other scar-specific therapy.

A Very Common Confusion
Acne Marks Are Not Always Acne Scars
A flat red, pink or brown mark left after a breakout is not the same thing as a pitted scar. Post-inflammatory erythema and post-inflammatory hyperpigmentation involve color. Atrophic acne scars involve a change in the physical architecture of the skin.
This distinction matters because pigment treatments and vascular treatments may improve discoloration without changing a depression, while collagen-remodeling procedures can improve texture without necessarily being the best treatment for lingering color.
Many acne-scar patients have both, so we often separate the treatment plan into texture and color.

Improvement — Not a Promise of “Scar Erasure”
People often search “acne scar removal,” but true acne scars are remodeled rather than literally erased. Treatment can soften edges, reduce depth, improve shadowing and make texture look substantially smoother.
The most realistic endpoint depends on scar depth and type. Deep scars usually need more than one treatment and sometimes more than one modality.
Acne Scar Treatment Options
CO₂ Laser, RF Microneedling, SkinPen & ResurFX
Each modality reaches the skin differently. The best treatment is the one that matches the scar architecture without creating unnecessary recovery or pigment risk.
Tetra CO₂ Fractional Laser
Fractional CO₂ is our most corrective resurfacing option for selected depressed acne scars. Controlled columns of ablative treatment resurface scarred skin while surrounding thermal effect stimulates progressive collagen remodeling.
The advantage is strength: CO₂ can address pronounced texture and scar edges while resurfacing the surrounding skin. The tradeoff is more visible recovery and greater need for careful treatment selection.
Virtue RF Microneedling
Virtue RF combines controlled microneedling with radiofrequency delivered below the skin surface. The needles create a mechanical repair signal while RF adds controlled thermal remodeling.
This makes Virtue especially useful when acne scars are accompanied by enlarged pores, uneven texture or loss of firmness. Treatment depth can be selected according to the scar pattern and tissue.
SkinPen Microneedling
SkinPen is collagen-induction therapy without RF energy or laser heat. Fine sterile needles create controlled microchannels that stimulate the skin's wound-healing response and new collagen.
SkinPen can be an excellent lower-downtime option for selected acne scars and uneven texture, particularly when aggressive resurfacing is unnecessary.
ResurFX 1550 Fractional Laser
ResurFX is a non-ablative fractional 1550 nm laser. Unlike CO₂, it creates fractional dermal treatment without ablating the skin surface, making it a lower-recovery laser option for selected scar and texture concerns.
It can fit patients who want fractional laser collagen remodeling but do not need — or do not want — the recovery associated with more aggressive ablative CO₂ resurfacing.
Professional Chemical Peels
Peels are generally not the first choice for deep pitted scar architecture, but they can improve superficial texture, acne-related discoloration and overall tone around scars.
They can also be useful in a staged plan when post-acne pigmentation makes scars look more noticeable even after the texture itself has improved.
Regenerative Adjuncts
Selected growth-factor, exosome/PDRN or PRP-type adjuncts may be incorporated into certain scar procedures to support recovery and skin quality. They should not be confused with the procedure that physically creates scar remodeling.
The scar-correction work still comes from appropriately selected microneedling, RF or laser treatment. Regenerative products are supportive rather than a substitute for reaching the scar at the appropriate depth.

The plan may also change as the skin improves. Once deeper texture is softened, the priority may shift toward pigmentation, pores or maintenance rather than repeatedly using the strongest treatment.
